We are very excited to announce that South Western City Schools has partnered with Final Forms, an online forms and data management service. Final Forms allows you to complete and sign athletic participation forms for your students. The most exciting news is that Final Forms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! Final Forms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. Does your child plan to play sports or participate in the marching band? There is scholarship money made available through the Success Beyond the Classroom Foundation (SBC) to assist in covering the Pay-to-Participate cost. SBC is a non-profit organization that raises funds to provide scholarships to qualified students in the district who wish to participate in athletics and/or their school marching band. SBC members believe that extra-curricular activities help students learn and practice the skills needed for success in all areas of life. To be considered for an SBC scholarship, students must submit and fully complete an application, which is available on the district and school websites. Administrators, Athletic Directors, and/or coaches at each building can help you locate the SBC application. Applications can also be found on the building and district websites. In order to participate in informal activities - open gym, conditioning, workouts- a student must have either an up to date physical on file with the office or a competed waiver form for their specific sport. Links to sport specific waivers can be found below, please make sure to complete them and bring them to the first day of workouts. Once the official season begins, students will need their completed physical to be able to participate/try out.
